WHEN IS PGD/PGS APPLIED?
PGD (Preimplantation Genetic Diagnosis) is applied to families in which one or both parents have a proven genetic abnormality (a chromosomal abnormality, a monogenic defect or a hereditary disease in the family transmitted through the X chromosome) and the embryo is tested for the particular mutation.
In PGS (Preimplantation Genetic Screening), embryos of parents with a normal karyotype are screened for aneuploidy (incorrect number of chromosomes). Indications for PGS are advanced reproductive age of the woman, multiple miscarriages, failed attempts from previous IVF procedures, severe male infertility.

what is PGD?
As the earliest form of prenatal diagnosis, PGD detects abnormalities in the fetus before a clinical pregnancy has occurred. In this way, the potential termination of pregnancy is avoided. This enables the couple to start a pregnancy with the knowledge that their child will not be affected by a genetic disease.
The main purpose of preimplantation genetic diagnosis is to test the chromosomes from the egg and sperm. If they do not find mutations in the genes, the embryos are euploid. By condition, each must contain 46 correct chromosomes. If the geneticists conclude that there are one or more healthy embryos, a transfer can proceed. In case some are unusable, they can be destroyed or frozen. The judgment rests with the patients and their reproductive specialist.
Genetic testing shows the number of chromosomes in the embryo.
The possible outcomes are broadly divided into three categories:
- Euploid embryo – correct number of chromosomes. This embryo is a priority for transfer.
- Aneuploid embryo – incorrect number of chromosomes. Such an embryo is avoided to be used.
- Mosaic embryo - in which part of the cells have the correct number of chromosomes, and another part has an incorrect number. For this type of embryos, depending on the severity of damage, it is assessed whether and in what order (if there are other embryos) it can be returned to the woman.

whan can PGD be applied?
PGD can be applied both to patients with infertility and to those who do not have reproductive problems, but the following factors are present:
- Repeated spontaneous abortions with an established abnormality in the fetus.
- More than three unsuccessful in vitro attempts with very good embryos on day 5.
- Anamnestic data on a child born with chromosomal disorders.
- Family burden – conditions related to structural disorders in chromosomes (e.g. translocations, inversions).
- Family burden related to the X-chromosome.
- Hereditary genetic abnormalities.
- Predisposition to certain cancers, Alzheimer's, etc.

The method helps to:
- Birth of a healthy child.
- Conception and development of a fetus without certain genetic and other disabilities in couples with an advanced childbearing age of the woman - the so-called "maternal age", which is considered advanced over 35 years.
- Success from consecutive invitro experience in couples with multiple previous failures from assisted reproductive techniques.
- Reducing the risk of spontaneous abortions, early or late pregnancy loss.
- Possibility to study the whole genome of the embryo on the given chromosomes.
What test do you need?
Not all couples undergoing in vitro fertilization are advised to do this type of testing.
