when is in vitro fertilization applied?
The in vitro fertilization procedure is used in:
- Tubal sterility.
- Unexplained sterility treated without success for 1.5 years in women under 35 and 6 months in women over 35.
- Male sterility.
- Endometriosis.
- Luteinized unruptured follicle (LUF) syndrome.
- Women after 4 inseminations, who did not end up with a pregnancy, etc.

What are the steps of the traditional procedure?
Doctors usually say that the main symptom of infertility is the absence of pregnancy after regular sexual intercourse for at least 12 months.
The steps of the traditional in vitro procedure are:
- Preparation
- Stimulation
- Puncture
- Fertilization
- Culturing of embryos
- Embryo transfer
- Pregnancy and pregnancy test
Intrauterine insemination (IUI)
Intrauterine insemination (IUI) is the placement of processed seminal fluid into the woman’s uterine cavity using a special thin and flexible catheter. Placement of the processed seminal fluid in the uterine cavity of the woman takes place a few hours before or after ovulation.
Indications for applying intrauterine insemination with processed spermatozoa from the husband (partner):
- Unexplained sterility.
- Mild subfertility in men.
- Presence of a bad postcoital test.
- Presence of sperm antibodies in the man and/or the woman.
- Ejaculatory disorders in men (erectile dysfunction, retrograde ejaculation, traumatic spinal cord injuries, etc.).
- In women after ovulation induction.
ICSI fertilization
ICSI fertilization is the intra-cytoplasmic injection of a sperm into the egg and is a variant of in vitro fertilization. This is a specific assisted reproductive technology (ART) technique.
ICSI involves selecting the most powerful, single sperm with the best motility and injecting them directly into a fertile egg with a microneedle for assisted fertilization. It may be necessary to perform controlled ovarian hyperstimulation, which aims to stimulate the formation of multiple mature eggs.
Retrieving a larger number of eggs makes it possible to obtain more embryos in the subsequent laboratory fertilization, which significantly increases the chances of pregnancy. Stimulation is carried out by the administration of drugs according to a scheme. The embryo is then cultured in a sterile embryology laboratory until it reaches the day 5 embryo stage (blastocyst).
The embryos obtained after fertilization are grown between 3-5 days in laboratory conditions. Subsequent uterine transfer of the blastocyst allows continued development in the womb. ICSI is accepted as a safe and highly effective medical technology for men with male factor infertility such as unhealthy sperm, low sperm count and women with low egg count, thick egg shells or unhealthy uterus to have a successful pregnancy outcome.
The main application of the ICSI method is in male factor infertility:
- Low sperm concentration and/or poor morphology.
- Reduced sperm mobility.
- Lack of sperm in the ejaculate.
- The ICSI method is also applied in some cases of a female factor of infertility, such as the presence of antisperm antibodies.
ICSI is also commonly used in cases of cryopreservation of seminal fluid. When the time is right, the frozen semen is thawed and the appropriate fertilization technique is applied
The main difference between in vitro fertilization and ICSI fertilization is in the laboratory stage. In classic in vitro fertilization, the egg is inseminated in a special petri dish with thousands of the partner’s sperm. The ICSI procedure has the difference that with the help of a micromanipulator system, one sperm is selected and injected into the cytoplasm of the egg cell.
Допълнителни тестове
Your doctor may recommend some of the following tests:
For the woman:
- Hormones: FSH, LH, E2 (tested on day 2-3 of the menstrual cycle), AMH (Anti-Mullerian hormone).
- TSH, FT4, TAT, MAT and PRL.
- Vitamin B12.
- Insulin resistance.
- Virology (AIDS, hepatitis B and C, syphilis) - it is recommended that the examination be done immediately before the examination date.
For the man:
- Spermogram – it is required to ensure sexual abstinence between 3 and 5 days.
- Virology (AIDS, hepatitis B and C, syphilis) - it is recommended that the examination be done immediately before the examination date.

Your reproductive specialist may order additional tests when there are co-morbidities that may affect the success of the in vitro procedure. Your treating doctor will inform you about the sequence of the stages of the procedure and will discuss with you each step that lies ahead.
